Abstract

Hypothesis / aims of study
To detect the postoperative complications and outcome of microscopic varicocelectomy in comparison of laparoscopic varicocelectomy.
Study design, materials and methods
This is a cross sectional Hospital based study was conduct in Sudan at Khartoum teaching hospital and Hawa infertility center during the period from April 2021 – April 2022. The recorded data of 42 patients who were complaining of infertility due to varicocele and underwent surgery (  half of them underwent microscopic varicocelectomy and the other half were underwent laparoscopic varicocelectomy) was gathered and analyzed  using SPSS.
Results
-After both types of operations all of the patients have no abnormality detected in their semen analysis. 
-Regarding the patients who underwent microscopic varicocelectomy:
1- Patients who had developed postoperative testicular pain:
There were three patients who had postoperative pain, one of them was around 25-30 years old, and rest were between 35-40, all of them were smokers, and had bilateral affected side. 
2- Patients who developed postoperative hydrocele: 0%
3-Those who developed recurrence within one year were two, one of them was 25-30 years old with bilateral affected side the other one age is between 30-35 with left affected side. Both of them were smokers.

-Regarding the the patients who underwent Laparoscopic Varicocelectomy: 
1- Patients who had developed postoperative testicular pain:
They were seven patients. Two of them were between 25-30 years old, four of them were between 30-35 while the rest were between 35-40 years old. Two of the patients had bilateral affected side while the rest had left affected side. Three of the patients were smokers. 
2- Patients who developed postoperative hydrocele:
 Two patients had developed postoperative hydrocele, one of them was a smoker 25-30 years old with bilateral affected side. The other one was 30-35 years old with bilateral affected side. 
3- Those who developed recurrence within one year were six, three of them were in the age between 25-30, two were between 30-35 while the rest were between 35-40. Two patients had bilateral affected side while the rest had left affected side and three of the patients were smokers.
Interpretation of results
From 21 patient whos underwent microscopic varicocelectomy we have just three patients who developed postoperative testicular pain in compared to seven patient of 21 whos underwent  Laparoscopic Varicocelectomy which is nearly to study was in Qatar a which stated that four patients from the total of 60 developed postoperative testicular pain in compared to six patients respectively.(1)
 From 21 patient whos underwent microscopic varicocelectomy we have no one whose developed postoperative hydrocele which is equal to study was presented at the 24th World Congress of Endourology, Cleveland, Ohio, August 17–20, 2006. in compared to two patient of 21  whos underwent  Laparoscopic Varicocelectomy. (2).
From 21 patient whos underwent microscopic varicocelectomy we have just two patient who developed recurrence within one year  which is nearly to study was in Egypt in in compared to three patient of 21  whos underwent  Laparoscopic Varicocelectomy.
Concluding message
According to the presented results and regardless to preoperative status of the patients, all who were underwent the Microscopic Varicocelectomy developed less postoperative complications than those who underwent Laparoscopic Varicocelectomy ; therefore, should be the gold standard operation for surgical treatment of varicocele
