Iatrogenic injuries, pelvic trauma, long term indwelling urethral catheters can result in a destroyed female urethra. Impaired bladder emptying and/ or total urinary incontinence can severely affect the quality of life of affected patients. While the severity of urethral destruction many vary, the completely destroyed female urethra will present a serious reconstructive challenge. The goals of the reconstruction are to allow unobstructive bladder emptying while maintaining urinary continence. Currently there are two schools of thought: one is to abandon the female urethra and close it either transvaginally or abdominally and perform either a continent cutaneous or an incontinent urinary diversion, the other is to use current female urethral reconstructive techniques to restore the length and patency of the female urethra while offering an anti-incontinence procedure. Dr Ockrinm will be debating on the side of the need to abandon a destroyed female urethra while Dr Gousse will debate the possibility to salvage the destroyed female urethra using various reconstructive techniques.
